If you are already registered for the Patient Portal, you can visit the Patient Portal page to log in.

To update your details, including but not limited to your address and insurance information, please download and complete the Patient Information Update form.

Once you have completed your form, you can either bring this information with you to the orthopedic office where you have your appointment or you can email it to us. Thank you!

All new prescriptions are given to the patient at the time of their visit. Requests for refills of medications should be made during office hours when your medical records are available. Please contact your local pharmacy and have them submit a refill request to our office. Refill requests are approved during regular business hours Monday – Friday. Please allow a 24-hour turnaround time. Refills requested on Friday afternoons may not be filled until Monday. At times, a refill may not be given if you have not had an office visit in more than six months.

In general, fees charged are comparable with those charged by other orthopedic surgeons in this area. Co-payment is expected on the day of service for office visits and surgical co-payments are collected prior to scheduling surgery. Monthly statements are mailed to each patient.

You should feel free to request an estimate of the charges for any surgery when scheduling a surgery date. It may not be possible to quote an exact figure. A specified period of postoperative care will be included in the charge for your operation. This includes routine postoperative treatment directly related to the surgery but not X-rays, cast changes, or unrelated conditions which might arise.

Effective June 1, 2022, Golden State Orthopedics & Spine no longer accepts cash. We continue to accept all major credit cards and checks. We accept Visa, MasterCard, and American Express. We appreciate your understanding.

If you need to cancel or reschedule an appointment, please do so at least 24 hours in advance. There is a $25 charge if an appointment is not cancelled within 24 hours. For your convenience, we have an automated system that reminds you of your appointment in advance.

Get a handle on your healthcare costs by knowing what you’re paying ahead of time. Transparent self-pay options are especially suited for patients with high insurance deductibles, with out-of-network insurance, or without insurance. We want to provide you with excellent service when you need a copy of your records and imaging studies. To request copies of your medical records and/or images, please complete and submit our Medical Records Transfer Request form.
